2024 EmblemHealth

EmblemHealth Platinum Premier

This is a good plan if you receive frequent care because there is a low in-network deductible, three (3) no-cost primary care provider (PCP) and telemedicine visits, and $5 generic drugs. This point-of-service (POS) plan provides in-network coverage on the Select Care network and out-of-network coverage at a higher cost.

In-Network Deductible: $100 individual / $200 family 

EmblemHealth Gold Premier

This is a good plan if you need medical care occasionally. It has a lower in-network deductible and offers three (3) no-cost PCP visits, no-cost telemedicine visits, and no-cost generic drugs before deductible. This point-of-service (POS) plan provides in-network coverage on the Select Care network and out-of-network coverage at a higher cost. 

In-Network Deductible: $500 individual / $1,000 family 

EmblemHealth Silver Premier

This is a good plan if you only need basic care but want some upfront benefits. It includes one (1) no-cost PCP visit, no-cost telemedicine visits, and $20 generic drugs before deductible. This point-of-service (POS) plan provides in-network coverage on our Select Care network and out-of-network coverage at a higher cost.

In-Network Deductible: $5,600 individual / $11,200 family 

EmblemHealth Silver Plus HSA

This high-deductible health savings account (HSA)-compatible Silver plan allows you to save pre-tax dollars to use toward medical care. This plan is on the Select Care network.

In-Network Deductible: $3,500 individual / $7,000 family 

EmblemHealth Bronze Premier

This is a good plan if you don’t seek care often but want some upfront services. It includes one (1) no-cost PCP visit, no-cost telemedicine visits, and generic drugs before deductible. This plan is on the Select Care network. 

In-Network Deductible: $7,100 individual / $14,200 family 

EmblemHealth Bronze Plus HSA

This Bronze HSA compatible plan allows you to save pre-tax dollars to use toward medical care. This plan is on our Select Care network.

In-Network Deductible: $7,400 individual / $14,800 family 

2023 EmblemHealth Prime

EmblemHealth Platinum Premier-P

This is a good plan if you receive frequent care because there is a $0 deductible, 3 free PCP visits, free generic drugs,and  free telemedicine and acupuncture. This plan is on our broad Prime network with Tristate access.

Deductible: $0 individual / $0 family 

EmblemHealth Gold Premier-P

This is a good plan if you need medical care occasionally. It has a low deductible with multiple services before deductible, 3 free PCP visits, free generic drugs, and free telemedicine and acupuncture. This plan is on our broad Prime network with Tristate access.

Deductible: $500 individual / $1,000 family

EmblemHealth Silver Premier-P

This is a good plan if you only need basic care but want some upfront benefits. This plan includes multiple services before deductible, 1 free PCP visit, free generic drugs, and free telemedicine and acupuncture. This plan is on our broad Prime network with Tristate access.

Deductible: $4,800 individual / $9,600 family

This is a high deductible Silver plan with access to a health savings account (HSA). An HSA plan allows you to save pre-tax dollars to use toward medical care. This plan is on our broad Prime network with Tristate access.

Deductible: $3,500 individual / $7,000 family

EmblemHealth Bronze Premier-P

This is a good plan if you don’t seek care often but want some upfront services. It includes 1 free PCP visit, generic drugs before deductible, and free telemedicine and acupuncture. This plan is on our broad Prime network with Tristate access. 

Deductible: $6,300 individual / $12,600 family

EmblemHealth Bronze Plus HSA

This is a high deductible Bronze plan with access to a health savings account (HSA). An HSA plan allows you to save pre-tax dollars to use toward medical care. This plan is on our broad Prime network with Tristate access.

Deductible: $6,750 individual / $13,500 family
